Position Description Position Summary:

The Transition Coordinator is a Certified Diabetes Educator who will coordinate care and liaise with Type 1 diabetes patients enrolled in a clinical trial during the transition period between paediatric and adult care. This is a five (5) year randomized control trial of 62 patients where approximately half receive counseling at 6 visits over 18 months and on-going coordination between visits. Responsibilities may include, but are not limited to, providing patient/participant education, maintaining contact with the patient via the subject’s preferred means of communication (phone, email, text messaging), sending reminders for patient/participant appointments, rescheduling missed appointments, ideally within 4 weeks, providing support with medical care under medical directive (insulin adjustments).

The Transition Coordinator collaborates with the principal investigator, all site specific co-investigators and research staff, and the respective health care teams (Paediatric Diabetes Clinic, Laboratory Services and Adult Diabetes Clinic) to ensure that all study assessments and milestones are completed. All duties of the Transition Coordinator must be performed according to the International Conference on Harmonization/Good Clinical Practice (ICH/GCP) guidelines, Health Canada Regulations and The Credit Valley Hospital and Trillium Health Centre’s research policies and standard operating procedures as applicable.


Key Qualifications:
• Diabetes Educator Certification
• Minimum of 3 years experience in diabetes education
• Previous experience in individual counselling, preferably with an adolescent population
• Previous research and/or clinical trials experience an asset
• Proficient with applicable computer software (Microsoft Word, Excel)
• Knowledge of clinics, hospitals, department procedures and policies preferred
• Communication of empathy, warmth and respect for participants
• Demonstrated excellent organizational skills
• Excellent oral and written communication skills
• Highly motivated and self-directed, with excellent interpersonal skills
• Ability to maintain subject confidentiality
• Positive attendance record
• Completion of the Tri-Council Policy Statement 2 (TCPS 2) Course on Research Ethics (CORE) or completion within 3 months of filling the position.
