Position Title Coordinator, Client Services

Job Summary The Coordinator, Client Services is responsible for carrying out the daily operations of Community Support Services in accordance with VON standards.  The Coordinator assesses clients for services and monitors service delivery, as well as contributes to the development, evaluation and promotion of Community Support Services.

The successful candidate will be on-call once every four weeks for the Meals on Wheels program. This is subject to change based upon business needs.
  • Determines client eligibility for services
  • Explains the service to prospective clients, completes a client assessment through a home visit or phone assessment, and establishes a client record and service plan including goals and outcomes
  • Ensures assessment process is collaborative and takes in the needs of the client and the caregiver, where appropriate
  • Recommends alternate and/or additional resources to the client, where required
  • Regularly monitors and revises the client service plan, as required
  • Reviews client records and ensures compliance to established procedures
  • Identifies client issues requiring discussion with a referring health professional and other caregiver and follows through accordingly
  • Participates in the development and implementation of program evaluation tools
  • Assists with data collection for program evaluation
  • Coordinates service provision with other site services, where appropriate
  • Prepares service summary report and statistics to Manager or designate
  • Participates in research, as requested
  • Responds to all telephone calls and inquiries with appropriate and timely interventions
  • May be responsible for supervision of staff, including administrative and transportation members
  • May be required to join the Community of Practice committee and other VON committees
  • Performs additional functions as delegated by the Manager, Community Support Services
  • Diploma in Health Care, Social Services or equivalent
  • Two years of demonstrated experience and special interest in Volunteer Management or Coordinator role
  • Warm manner and genuine interest in people that will inspire confidence on the part of professionals, clients and volunteers
  • Personal suitability including the ability to exercise initiative, tact, discretion & judgment
  • Ability to work collaboratively as part of a team, as well as to work independently with minimal supervision
  • Able to deal effectively with people and work under pressure
  • Excellent oral and written communication skills; listening and public speaking skills
  • Ability to be adaptable and flexible, highly motivated and able to work independently
  • Possess good judgment and decision-making skills, including during emergency situations
  • Excellent planning, organizational and assessment skills
  • Able to adapt to change & work flexible hours
  • Excellent computer skills (Microsoft Word, Excel, PowerPoint)
  • Ability to quickly learn and work with a computerized Database Management software
  • Willingness to travel to neighboring communities
  • Must have personal vehicle and possess both a current driver’s license and proof of vehicle insurance
  • Current criminal background check will be requested
