Job Summary
The successful applicant will provide safe, client-centred health care in the community using the nursing process, scope and standards of clinical practice established by VON and the College of Nurses of Ontario and principles of quality management.
They will also have the opportunity to contribute to VON’s responsibility for developing programs to meet community health needs.
The successful applicant will provide joint visits with nursing staff to palliative clients to coach and mentor which will improve the quality of care to clients and their families. They will also provide independent visits when required for assessment and to set up care plans for palliative clients.
The successful applicant will attend CCAC PCCT meetings on behalf of the VON Windsor-Essex and VON Chatham-Kent sites and PCCT on-call for one site.
This position will include travel to both VON Windsor-Essex and VON Chatham-Kent regions, with the potential to expand to the VON Sarnia-Lambton region in future.
  • Provide and manage high quality, appropriate and cost effective nursing care to all individuals and/or families in the community who are assigned to their care
  • Responsible to teach and delegate tasks and functions to different levels of workers and to family members and others in accordance with the standards and scope of practice as determined by VON and the College of Nurses of Ontario
  • Comply with all standards, policies, procedures and legislative requirements
  • Participate as a member of the health care team in relevant committees and working groups
  • Current registration as a Registered Nurse in the province of Ontario
  • Must have CNA-Canadian Hospice Palliative Care certification and CAPCE
  • Minimum of 5 years recent palliative care experience
  • IV and CVAD proficiency
  • Proven experience in [palliative] mentoring and education in order to enhance scope of practice
  • Demonstrated skills in observation, independent assessment, health teaching, communication and interpersonal relationship building
  • Good oral and written customer service and communication skills
  • Critical thinking skills and problem-solving abilities
  • Ability to speak and/or understand a second language is desirable
  • Previous experience within a community care setting is preferred
  • Understanding of healthy lifestyles and ability to advise
  • Advanced preparation, certification or skill training is preferred
  • Current criminal record check
  • Valid driver’s license, insurance and access to a reliable vehicle
