||The VON 360 Degree Nurse Practitioner- Led Clinic is located in the downtown core of Peterborough and serves the primary health care needs for the residents of Peterborough city and county who are unattached and especially the underserved or marginalized population. The successful candidate will be a member of a interdisciplinary team providing comprehensive, collaborative and client focused care. The VON 360 provides “wrap-around” services through referrals and partnerships with area providers and includes a focus on health promotion and disease prevention. The Registered Nurse is responsible to provide excellent direct nursing care services to clients in a primary care clinic setting.
||• Provides direct nursing care services to clients • Assists with coordination of health care information • Maintains accurate electronic client clinical records • Communicates with community partners as appropriate • Participates fully as member of interdisciplinary team • Provides leadership in own professional development • Participate in CNO Reflective Practice • Adheres to the policies and procedures of the VON 360 • Participates in outreach activities as assigned by NP Lead • Other duties as assigned by NP Lead
||• Registered with College of Nurses of Ontario, B.Sc.N preferred • Recent experience working in community based health care system • Ability to work independently and collaboratively as a member of intra/inter agency teams • EMR (electronic medical records) experience is an asset • Phlebotomy skill is an asset • Foot care certification an asset • Ability to provide leadership within the organization • Current Criminal Records Search
