Position Title Delegating Registered Nurse - Palliative

Job Summary The successful candidate for this role will manage palliative care clients as "delegators" working remotely, monitoring, mentoring and directing care delivery. The DRN will provide support and training for Personal Support Workers - Palliative who are providing palliative care to clients within the same program
  • Provide excellent direct nursing care services to clients and families in their homes
  • Maintain accurate client clinical records
  • Communicate with Case Managers as appropriate
  • Participate as full member of the health care team
  • Provide leadership in own professional development
  • Participate in CNO Reflective Practice
  • Knowledge of VON policies and procedures
  • Provide clinical leadership within district
  • Registered and in good standing with the College of Nurses of Ontario
  • CAN Hospice Palliative Care Certification or Fundamentals is required
  • CAPCE program completion is an asset
  • Current CPR and First Aid certification will be requested
  • Experience in palliative care is required
  • Experience in gerontology and/or medical-surgical is an asset
  • Experience working in community based health care system is an asset
  • Demonstrated competencies in observation, independent assessment; health education and promotion
  • Proficient nursing skills with the ability to read and interpret technical and clinical instructions
  • Critical thinking, problem solving and multi-tasking skills are mandatory
  • Ability to work in a multidisciplinary team setting, as well as the ability to work independently is required
  • Must have a home computer, basic computer skills and the ability to work from home
  • Must have intermediate-to-advanced cellular telephone/iPhone/BlackBerry skills
  • Must have personal vehicle and possess both a current driver’s license and proof of vehicle insurance
  • Individual must be eligible and successfully obtain a security screening certificate from the Government of Canada
Work Conditions and Physical Capabilities: Fast-paced Environment; Attention to Detail; Lift and carry using appropriate lifting techniques; Bend, push, pull, grip, reach, kneel, crouch using good body mechanics; Walk, sit, stand, climb stairs; Fine hand movements
Other Relevant Information As this is a remote, work-from-home position, consideration will be given to those candidates living within a one hour drive to the VON Brantford office. Scheduled meetings, etc will require occasional travel to the Brantford office.
