Position Title RPN Team Lead - Home Support
Program/Department Home Support
Salary $19.96 - $24.55
Job Summary
The successful RPN applicant will provide team lead support to the Manager, Home Support. Care is provided within the framework of quality management and the standards and scope of practice set by VON and the College of Nurses in Ontario.
  • Communicate with CCAC case managers regarding client status, service plan, service end dates and risk
  • Work with the Client Care Coordinator to form and maintain, cluster care teams
  • Perform Home Support Chart auditing monthly
  • Conduct the [PSW] first visit with clients
  • Perform interim client visits as directed by CCAC
  • Complete discharge reports
  • Provide training to new PSWs, including orientation
  • Provide excellent direct nursing care services
  • Maintain accurate client clinical records
  • Participate as a full member of the health care team
  • Provide leadership in own professional development
  • Participate in CNO Reflective Practice
  • Provide clinical leadership within district
  • Must be available to work flexible hours and to participate in on call rotation
  • Completion of a post-secondary Registered Practical Nursing (RPN) Diploma is mandatory
  • Registered with College of Nurses of Ontario
  • Current CPR and First Aid certification will be requested
  • Minimum three years experience working in a community based health care system
  • Experience in teaching PSWs or other health care members an asset
  • Palliative, pediatric, and wound care experience an asset
  • Knowledge of VON policies and procedures/Lean management system an asset
  • Familiarity and understanding or role of Community Care Access Centre (CCAC)
  • Knowledge and experience with CHRIS Portal and Procura scheduling an asset
  • Advanced computers skills required, including Microsoft Office Suite and Windows OS
  • Ability to provide leadership within the organization
  • Ability to work effectively in a multidisciplinary team, take direction well and work independently
  • Possess relationship and team building skills
  • Must be able to communicate well, both written and verbal
  • Must have personal vehicle and possess both a current driver’s license and proof of vehicle insurance
  • Current criminal background check will be requested
