Position Title Team Lead, Home Support Program

Job Summary
As Team Lead, Home Support Program, you will be responsible for the provision and supervision of cost effective homecare to individuals and/or families in the community. Care is provided within the framework of quality and risk management and the standards and scope of practice set by VON and the CNO. You will be responsible for building high client satisfaction outcomes and coaching and supporting an effective service delivery team by providing education, leadership and support to team members to meet CCAC contract standards of care.
  • Meet with individual team members to build education and learning plans and gives input to annual PDP.
  • Provide complete, relevant assessment of the client/family and their environment utilizing established procedures.
  • Collaborate with the service delivery team and contribute to the delivery of the client’s care plan.
  • Supervise the work of the team according to planning guidelines and geographical boundaries.
  • Work collaboratively within a multidisciplinary team and acts as a clinical resource and mentor for other care providers, staff and students.
  • Meet with individual team members to build education and learning plans and gives input to annual PDP.
  • Act as a role model to the team by demonstrating professionalism and building exceptional client experience in all interactions.
  • Ensure optimal care to clients by maintaining confidentiality and acting in an ethical and professional manner, in accordance with the professional code of ethics.
  • Function and report in compliance with the Occupational Health and Safety legislation, regulations, and VON policies and procedures.
  • Certificate of Competence/License as a Registered Practical Nurse in the Province where service is delivered.
  • Minimum of 3 years RPN Clinical experience.
  • Hands on personal care experience in community health, an asset.
  • Team lead or supervisory experienced.
  • Experienced in the use of Microsoft Office.
  • Strong observation and assessment skills.
  • Health teaching and facilitation experience.
  • Strong verbal and written communication skills.
  • Interpersonal relationship building.
  • Ability to problem solve and adapt nursing/unregulated service provider interventions as appropriate.
  • Ability to work collaboratively as a member of an interdisciplinary team.
  • Coaching, mentoring and conflict resolution skills.
