Position Description Key Qualifications:      
 
Registered Social Worker in good standings with the College of Social Workers  of Ontario
Masters in Social Work required
Experience in program planning, development, implementation, monitoring, management and evaluation at the community level
Excellent assessment and telephone skills in the area of adult mental health
Knowledge of Trillium’s Mental Health Services admission criteria
A minimum of 5 years of mental health experience in a primary care setting  required
Knowledge of the mental Health act
Working knowledge of community resources
Excellent interpersonal and communication skills and demonstrated ability to work independently and as part of an interdisciplinary team
Possesses the ability to function effectively during periods of rapid change and transition
Possess excellent oral skill
CNA certification in mental health nursing an asset
Ability to work in a fast pace environment
 
 
Position Summary:
 
Provide telephone assessments for all individuals referred to Trillium’s Ambulatory, Community, and Seniors Mental Health Services.
Attend meetings as required
Maintains client database and current inventory of community resources
Experience with the B-sharp database, excel, and Microsoft word 
Liaises and consults with community referrers, service providers, clients and families and participates in mental health planning
Work in a collegial relationship with the core team of the mental health system at all 3 hospital sites, along with family physicians and other external community agencies. including community care access centres, public health and community health centres regarding primary health care issues
Contributes to program planning
Maintain current health profiles for Trillium communities
