Position Description

 

Job Description:
 
  • Act as the sole conduit for patient referral from any source.
  • Book the patient appointment to the appropriate Oncology resource as per policy
  • Collate all personal health information needed for the appointment from internal and external sources in accordance with established protocol.
  • Follow up with internal/external sources for missing information.
  • Communicate the appointment to the patient and/or referring physician as per guidelines.
  • Initiate patient education re visit as per Oncology policy.
  • Monitor waiting times for appointments and notify responsible person within the Oncology Program when timeline standards are not achieved for an individual patient.
  • Ensure that the status of all patents referred to the Oncology Program can be tracked and reported within reporting guidelines at any time.
  • Initiate data gathering (mainly demographic) from the patient
  • Register patients accurately without creating duplicate health records number.
  • Ensure all reports are incorporated into the patient’s health record.
  • Ensure that the information required for NACRS is complete and accurate.
 
Qualifications:
 
  • Minimum OAC
  • Graduate of a community college course in medical terminology
  • Previous hospital/physician’s office experience
  • Computer trained in Microsoft suite of products
  • Excellent communication and organizational skills
  • Must be extremely detail and quality oriented
  • Meditech experience required
  • Satisfactory attendance and performance required
