-Establish continuity of care and navigate the patient’s care throughout the systemic treatment phase in conjunction with the Primary Nurse and/or the Radiation Oncology RN, Clinical Trials RN and the interdisciplinary team

-During the treatment course and immediate follow-up, assess and provide appropriate nursing intervention for actual or potential problems that the patient and family may experience related to the disease process and treatment

-Provide ongoing knowledge and information to patient and family to enable them to prevent and manage common side effects of treatment

-Use CCO telephone triage guidelines when practicing telephone triage responsibilities

-Graduate of a recognized college or school of nursing; BScN preferred

-Chemotherapy Certification; CNA certification; CON(c)

-Certification in I.V. and CVAD management

-Current BCLS

-Demonstrate general knowledge about Radiation Therapy, Surgical Oncology, Clinical Trials and Palliative Care

-Computer skills an asset

-Satisfactory attendance and performance are required
