Position Description


Position Summary:
The Nurse Practitioner (NP) is a Registered Nurse Extended Class with graduate education in nursing, possessing advanced clinical and theoretical knowledge/skill within the clinical specialty. The NP is a professional who will function in an expanded role and will demonstrate a high level of autonomy as well as expert skill to formulate clinical decisions, manage acute/chronic illness, and promote wellness.
In this position, the NP will provide service to Geriatric Assessment Clinics and the Regional Geriatric Medical Outreach Service. The NP will: conduct comprehensive geriatric assessments; diagnose; provide treatment (including diagnostic, pharmacological and non-pharmacological therapies and counseling); develop, implement and document a plan of care in collaboration with the geriatricians and interprofessional team; provide education; refer to/liaise with other community services; and, provide follow-up as appropriate. In addition, the NP may be involved in other clinical, research, evaluation, leadership and collaborative activities (e.g. developing new standards of care, policies, etc.).
The Principal Responsibilities include:
·         Combining the roles that fall within the domains of advanced nursing practice including clinical practice, research, leadership, collaboration
          and consultation.
·         Developing effective partnerships with physicians and other members of the inter-professional team to provide quality care to patients and
          their families within the area of gerontological nursing.
·         Applying a comprehensive theoretical knowledge base within the scope of practice in the extended class, the NP works collaboratively and
          independently to assess and manage patients’ geriatric medical care requirements, demonstrating exemplary clinical assessment and
          decision making skills.
·         Providing a key leadership role in evaluating quality of care and patient outcomes within the specialty population, in partnership with the
          physicians and other members of the inter-professional team.
·         Providing informal and formal educational opportunities (e.g. students, presentations, conferences, etc.).
·         Facilitating the research process.
·         Identifying quality initiatives that will strengthen and further develop nursing professional practice.
·         Evaluating the care provided to the patient including maintaining accurate statistics for reporting to the Mississauga Halton Local Health
          Integration Network (MH LHIN).
Key Qualifications:
·         Master’s Degree in Nursing with clinical focus in gerontology
·         Registration in the Registered Nurse Extended Class with the College of Nurses
·         Membership in the Registered Nurses’ Association of Ontario, required
·         Gerontological nursing practice, preferred
·         Canadian Nurses Association certification in gerontological nursing, an asset
·         Demonstrated experience with completing comprehensive geriatric assessments
·         Experience in community nursing preferred
·         Experience in evaluating quality of care and patient outcomes within the specialty population, in partnership with physicians and other
          members of the inter-professional team
·         Experience with research methodology, an asset
·         Eligible for clinical appointment to an academic institution
·         Teaching experience an asset
·         Demonstrated leadership skills
·         Strong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community
·         Current Basic Cardiac Life Support (BCLS) certification
·         Valid Ontario Driver’s License, relevant insurance and use of own vehicle as well as a willingness to travel throughout the Mississauga Halton
          Local Health Integration Network
·         Satisfactory attendance and performance required
