Position Description  

JOB PURPOSE AND SUMMARY

Reporting to the Vice President, Patient Care Services, this position brings a high level of operational expertise and leadership skills to a diverse care team. .The responsibilities of the Program Director, Primary Care, Rehab, CCC & Senior’s Health include ensuring the delivery of excellent and safe care for patients/families/clients in all portfolio areas and creating a healthy work environment for staff, physicians and volunteers. You will have operating accountability for a broad and diverse range of services and programs and will play a crucial role in achieving the organization’s vision, mission and strategic initiatives. In particular, Transitions from acute care to the community will play a key role in achieving our strategic plan. This role is pivotal in accomplishing that.

JOB DUTIES AND RESPONSIBILITIES

  • Plans, develops and ensures the delivery of programs in designated areas to provide patient and family-centred care in collaboration with internal management and community referral sources

  • Provides leadership for the development and implementation of the strategic planning process

  • In collaboration with our community partners, provides leadership in the delivery of providing Senior’s Care across the continuum.

  • Is aware of best practices for rehab and CCC, and strives towards achieving best practices in transitional care to improve patients outcomes, including their return to home.

  • Develops strategic alliances and partnerships with the business community and other referral sources and health programs

  • Monitors and oversees the fiscal operations of the Primary Care, Rehab, CCC and Senior’s Health areas

  • Prepares proposals and business cases for entrepreneurial and innovative ideas that enhances all potential revenues and service cost effectiveness

  • Provides leadership and direction for the Primary Care, Rehab, CCC & Senior’s Health portfolio management across the organization to achieve outcomes

  • Identifies trends and emerging needs within the patient population and maintains service responsiveness to community needs

  • Represents the organization on various Board committees for the family health teams when applicable

  • Contributes to the success of specialized programs such as the NP Stat Program

  • Promotes and advances best practices within the organization

  • Leverages external relationships with community providers and regional partners

  • Actively participates in committees, both internal and external, addressing collaboration and coordination of services

  • Engages with a multidisciplinary team of hospital professional staff, clinicians, service providers, contracts and others to ensure the quality delivery of all Programs

JOB SPECIFICATIONS

Knowledge, Experience and Skills

  • Masters degree in a health related field

  • 7+ years professional experience in a health discipline with progressive management experience

  • Must be a member in good standing of a regulated health profession

  • Clinical experience in the areas of Rehabilitation, Complex Continuing Care or Senior’s Health is an asset

  • Strong, well developed business management skills (strategic planning, budgeting, resource allocation and analytics)

  • Well developed, leadership and management skills

  • Excellent written and verbal communication and presentation skills

  • Results-oriented and client-focused with a commitment to excellence

  • Superior analytical, problem solving, negotiation and conflict resolution skills

  • Ability to build effective working relationships with a dynamic, professional team

  • Demonstrated management and staff development with emphasis on coaching, mentoring and leadership

     

Position Location and Travel:

Travel among the 3 main sites of The Credit Valley Hospital and Trillium Health Centre will be required.
